I authorize Talking Books Service by Mail to send me promotional information by phone or by mail. Occasionally, public libraries send out information to their patrons to promote new programs and services, special activities and events or information of local interest. Please note that this option will not affect the delivery of overdue notices, replacement bills or collection agency notices. This type of communication is related to your obligations when you borrow materials; therefore, you will receive these notices even if you have indicated that you do not wish to be contacted by the service.

Eligibility to Use Restricted Alternative Formats for People with Print Disabilities

Do you have a print disability due to a vision impairment, learning disability or physical disability in accordance with the definition provided for "perceptual disability" in the Canadian Copyright Act?

A visual disability: severe or total impairment of sight or the inability to focus or move one's eyes. 

A physical disability: the inability to hold or manipulate a book. 

A learning disability: an impairment relating to comprehension. 

Agreement

I acknowledge that I have a perceptual disability as defined under the Canadian Copyright Act and that I may be called upon to provide proof of such disability from a competent authority. Competent authority is defined to include doctors of medicine, ophthalmologists, optometrists, registered nurses, registered therapists, professional staff of hospitals, institutions and public agencies (e.g., vision rehabilitation specialist, social workers, case workers, counsellors, teachers and school district superintendents). A CNIB client number will be accepted as a competent authority. Library materials restricted to people with print disabilities (e.g. DAISY talking books) are for my personal use only. I will not copy, share or redistribute these materials.
